Physical Paperwork
Any student planning to participate in Athletics, JROTC, or Band for the 2024-2025 school year must have a completed physical on file by their first practice.
As a convenience, Central Catholic has a physical form below to be completed by a preferred physician. Families can also submit a standard physical form generated by a physician's office.
Physical Exams are being offered at the KCCC on Tuesday, May 21, 2024 beginning at 4:00 PM for a cost of $20. If you choose to have your son's physical done by your Family Physician, the completed physical exam must be emailed to Lanning Tucker at [email protected].
